Please select the time frame in which you would like to start your trial Groupon bootcamp membership. (subject to available spots in bootcamp classes. If desired start date is not available, you will be assigned the next closest start date. Class times subject to change without notice)
Billing Information
*TNT Bootcamp has a 24 hour cancellation policy. Due to the popularity and limitedness of bootcamp spots, once your start date has been confirmed and scheduled, you must provide us with at least 24 hours notice if you wish to reschedule otherwise resulting in a $10 cancellation fee.

I understand that TNT Bootcamp is an intense workout routine. I understand that by participating in bootcamp, TNT Bootcamp is not responsible or liable for any injury that may occur. By checking this box, I am agreeing that I am a healthy individual and have been cleared by my physician to participate in this activity.

I understand that my Groupon purchase is only good for bootcamp classes and class times are subject to change without notice.
I understand that in order to acheive the best results, I need to work hard and give 100% effort in class. I understand for best results I need to attend at least 3 days per week. I understand that although bootcamp can be challenging and may not be for everyone, if I give my best during class and eat right outside of class, I will see results. I also understand that if I am not willing to give 100% each class then bootcamp is not for me.
