I understand that TNT Bootcamp is an intense workout routine. I understand that by participating in bootcamp, TNT Bootcamp is not responsible or liable for any injury that may occur. By checking this box, I am agreeing that I am a healthy individual and have been cleared by my physician to participate in this activity.
I understand that TNT Bootcamp has a 24 hour cancellation policy. If I wish to reschedule my first initial class appointment, I must provide TNT bootcamp with at least 24 hours notice. I understand that in order to acheive the best results, I need to work hard and give 100% effort in class. I understand that although bootcamp can be challenging and may not be for everyone, if I give my best during class and eat right outside of class, I will see results.
